fixing a leaking bathroom extractor fan DIY

Water is leaking through my bathroom extractor fan. Can I fix this by myself and if so, what are the steps involved?

"Many extractor fans leak from poorly routed trunking through loft spaces. They allow water to pool rather then extract the steam. The solution is the correct extractor and ducting."
